Appearance Matters - Dental - Dentistry - Patient Pathway

Now that you are registered you are ready to begin your dental treatment and you'll no doubt be anxious to know what happens next. Below you will find an outline of your Patient Pathway, a nine-point plan that will detail every step of your treatment, from the minute we receive your instructions, to after you arrive home.

  1. Initial arrangements
    Based on information on your dental health (including provision of any necessary pan-oral x-rays, and/or bitewing x-rays and/or study models), your Confidential Medical Questionnaire and treatment preferences, Appearance Matters will ask the chosen Polish dental clinic to prepare a treatment plan consisting of:

    • Outline of dental treatment required

    • Duration of each phase of dental treatment

    • Proposed timetable of dental treatment

    • Preliminary estimate of costs

    • Suggested date(s) of your visit(s) to Poland

  2. Once the answers to the above have been received by Appearance Matters, we will ask you if you accept the:

    • Proposed treatment

    • Duration(s) and timetable

    • Estimated costs

    • Suggested dates

  3. As soon as your treatment arrangements in Poland are agreed, Appearance Matters will confirm with you:

    • That you and any persons travelling with you have booked appropriate travel arrangements and will obtain details of your arrival and departure dates and times.

    • That you and any persons travelling with you have valid passports and current travel insurance.

    • That you and any persons travelling with you possess a European Health insurance Card (EHIC) card (the replacement for the E111 form), which permits all holders to receive medical treatment in Poland under the Polish national health service (NFZ), in accordance with the same rules as Polish citizens.

    • That you and any persons travelling with you have made suitable accommodation arrangements or have requested Appearance matters to undertake such arrangements on your behalf.

    • Your interests and preferences e.g. choice of leisure activities, such as beauty therapy, massage, ballet, opera, museum visits and so on, so that Appearance Matters can forward any relevant and useful information regarding leisure time to you and can then pre-book any excursions or therapies on your behalf.

    • That you have requested Appearance Matters to provide you with an agreed amount of Polish currency on arrival (if required).

  4. A Welcome Pack, tailored specifically to your needs, will be prepared and available on arrival in Warsaw which will include:

    • A mobile phone with SIM card and pre-paid top-up (for use within Poland only).

    • Polish currency - the previously agreed amount between you and Appearance Matters.

    • A business card for your Appearance Matters Warsaw representative.*

    • Contact details for your Warsaw dental clinic.

    • The address and telephone numbers for your accommodation in Warsaw.

    • Taxi company telephone numbers.**

    • Warsaw A - Z map with location of your hotel and clinic clearly marked.

    • Weekly public transport ticket for you, the patient (if required).

    • Tickets for pre-booked leisure activities.

    • Postcard for your home dentist.

    • Warsaw tourist information leaflets including details of local events.

    N.B.
    *Please note that although normal business hours are from 8.00am to 6.00pm, your Appearance Matters Warsaw representative is flexible and will be available by phone 24 hours for emergencies.

    **Please also note that all taxi fares between the hotel and clinic in Warsaw will be paid by Appearance Matters. Please note that the taxi firm we use, MPT, is ISO 9000 certified and all MPT taxis are white.

  5. On arrival in Warsaw, Poland:

    • You will be met at the airport by your Appearance Matters Warsaw representative who will arrange for taxi transfer and will accompany you to your chosen hotel.

    • Your representative will provide you with your Welcome Pack, any additional material you have previously requested and a reminder of the timetable for your stay, including times of transfers to the clinic and times of treatment.

  6. On the first day of treatment:

    • You will be escorted from your hotel to the dental clinic by your Appearance Matters Warsaw representative by pre-booked taxi.

    • After treatment, the clinic will provide you with documented post-treatment recommendations, such as refraining from eating coloured food or drinking coloured drinks for a minimum 24 hours after tooth-whitening.

    • The clinic will arrange for a taxi to collect you and return you directly to your hotel.

    • After your return to the hotel, your Appearance Matters Warsaw representative will contact you to check that everything is OK, or if you require any specific assistance. S/he will remind you of the timetable for the following day, including taxi transfer to the clinic and schedule of treatment (if required).

  7. On subsequent days of treatment (if required):

    • You will be collected from your hotel by pre-booked taxi and will be escorted to the front door of the dental clinic by the driver.

    • After treatment you will be given documented post-treatment advice.

    • The dental clinic will arrange for a taxi to collect you and return you directly to your hotel.

    • After your return to the hotel, your Appearance Matters Warsaw representative will contact you to check that everything is OK, or if you require any specific assistance. S/he will remind you of the timetable for the following day, including taxi transfer to the clinic and time of treatment (if required).

  8. Day of departure from Warsaw, Poland:

    • Your Appearance Matters Warsaw representative will escort you to the airport for your return journey. S/he will be available to provide assistance until you have boarded the plane.

    • Your representative will collect from you the mobile phone, which you received on arrival, plus any other items from the Welcome Pack that you no longer require.

    N.B.
    Your hotel will usually be able to provide a packed breakfast if you have early morning flights.

  9. After your return home:

    • Appearance Matters will receive confirmation of the completion of your treatment, including the scope of treatment carried out and confirmation that you have received documented post-treatment recommendations for yourself and your home dentist.

    • Appearance Matters will contact you shortly after your return home to collect your Feedback on your stay in Warsaw.

    • You will be sent a reminder when your check-up consultation with your home dentist is due (if required).
